For context this affects Kaoxia drives on 26.04 with 7.0, but did not
affect the same Kaoxia drives on the same server model with 24.04 and
6.8?

Does this also happen on 25.10 (Questing) with 6.17?

I'm wondering where along the way this stopped working.

And to be sure, you have not updated firmware on the Kaoxia drives in
the meanwhile?

I'm escalating it and waiting for some advice, and seeing if I can find
some place to start looking.

one thing... can you retry this on 26.04 again, but don't reinstall all
the drives at the same time. PUll the drives out, but then re-insert
them with a bit of time between each one (30 - 60 seconds perhaps) to
see if it has to do with how quickly all four drives are reinserted?
