Current logs show PCI hotplug and NVMe controller initialization are mostly OK after replug:
nvme nvmeX: pci function ... nvme nvmeX: 128/0/0 default/read/poll queues However, after replug, nvme list / lsblk lose the namespace devices, and dmesg does not show an obvious NVMe namespace failure, timeout, reset failure, or I/O error. The missing clue is why namespace/block-device creation is not completed after the controller is ready.
