Can you run `apport-collect 2146770` to gather some logs for this on the failing system? Are you sure this is not related to the system, system FW, etc, bad cables, unconfigured or misconfigured SAN, etc?
Also, this same device appears to have been used in a few other Lenovo systems.. heres one: SR655 V3 https://certification.canonical.com/hardware/202303-31346/submission/486233/| Looking through dmesg and syslog on that one with the 7.0 kernel and 26.04 I do not see any indication of this messaging (dmesg is attached directly to the submission, syslog is via the sosreport attachment in the submission.
