This root cause analysis is courtesy of Google Gemini. I have no idea
if this is correct, but I've been plagued with broken icons in various
apps ever since I upgraded to Ubuntu 26.
I've attached a screen shot of the Kooha application, installed via apt
(not flatpak). The first is with the Yaru theme, and the second is with
Adwaita.
