I'm hitting the same crash on different hardware. Same call chain, same
fault address (0x219).

System: ASUS Vivobook S 16 M5606UA
Distro: Linux Mint 22.3 (Noble base)
Bluetooth device: MediaTek MT7922 combo card. WiFi via PCIe (mt7921e, 
14c3:0616). Bluetooth via USB (Foxconn/Hon Hai Wireless_Device, 0489:e0f5).

Confirmed regression: 6.8.0-138-generic works. 6.8.0-139-generic is
broken.

Result: hci0 stays registered but dead (BD Address: 00:00:00:00:00:00),
and bluetoothctl reports "No default controller available". Reverting to
6.8.0-138-generic fixes it completely.
