I can reproduce the “Unknown Bus Offset Setting (3)” failure on a Dell Pro Max 16 Plus MB16250, using Ubuntu 24.04 installed from Dell’s A01 recovery image.
BIOS: 3.3.2, storage mode: RAID On. Intel VMD controller: 8086:ad0b. Kernel 6.14.0-1015-oem boots successfully and detects both internal NVMe SSDs. Kernel 6.17.0-1032-oem drops into initramfs with no NVMe devices visible and reports: vmd 0000:00:0e.0: Unknown Bus Offset Setting (3) Both vmd and nvme modules are present in the failing kernel’s initramfs. Returning to 6.14 restores normal operation.
