Thank you for providing a complete report with all relevant log files. Typically with X crashes (and sometimes freezes), a backtrace is printed at the end of the Xorg.0.log, yet it did not in this case unfortunately.
Are you able to ssh into the machine after it has frozen? If so, then a good approach for troubleshooting would be to install -dbg packages for xserver and your driver, then via ssh attach gdb to Xorg and get a full backtrace. Steps on doing this are explained at https://wiki.ubuntu.com/DebuggingXorg.
