Public bug reported:
Testing in hardy alpha 3 (and earlier in 4) as well as Gutsy, all fail
in a similar way.
Device: AR2413 NIC (rev 01)
(also fails similarly with a BCM43xx card (Linksys WPC54G v3) and a
D-Link USB DWL-G133)

I will attach the dmesg which contains some fishy failures relating to
ath* driver as well as a verbose output from wpa_supplicant which
hopefully somebody can see where it is going wrong.
The AP is working fine with windows hosts and Gentoo Linux hosts using
other devices.
I do not think this is madwifi specific because I can not get it to work
with 2 other different wifi devices listed above.
