I'm unsure at the moment what is needed, from the above descriptions there are several different confusingly related problems going on. Sadly it appears that the vast majority of people do not see this, so it's working mostly right for them.
I've seen the /sbin/reboot command fail to actually reboot once myself, which made no sense because it *had* called the kernel reboot() system call.
