Public bug reported:

Binary package hint: gnome-panel

When I drag and drop an item from the "Application Launcher" section of
the "Add to Panel" window, instead of showing the application's icon on
the new launcher, it shows a yellow diamond with a question mark on it.
When I hover my mouse over it, it shows (using the example of
Calculator):

(null)
Open '/usr/share/applications/gcalctool.desktop'

When you add the launcher using the "Add" button in the "Add to Panel"
window, the launcher is added correctly.

Steps to reproduce:
1. Right-click an empty space in the panel and select "Add to Panel..."
2. Click "Application Launcher"
3. Drag an application to the panel

Expected result: The application button displays in the panel with the
application's icon, and the name of the application in the tooltip

Actual result: A yellow diamond with a question mark in it is displayed
in the panel, and the tooltip displays (null)
