There's several cases when this happens, which makes it look rather
random, but I can describe one case that is reproducible for me:

1) I have two windows open (1 firefox and one terminal, but it shouldn't matter)
2) I click the "show desktop" button on the gnome panel (1st time)
3) I open another window, for example nautilus by clicking the recycle bin in 
the gnome panel

status now: 
One open nautilus window and two window corners shown one each in the top and 
bottom right screen corner.

4) I click "show desktop" again.  (2nd time)

status:
Nautilus stays open, the other windows come back into the background.

5) click "show desktop" again (3rd time) --> all windows move into corners (as 
expected)
6) click it again (4th time):

Result:
Recycle window (nautilus) vanishes completely - from screen and window selector.

Expected: 
All 3 windows come back from the corners.

I am trying this over and over again, while writing the report, and it keeps 
working  that way.
The process of the lost window continues to run. Only the window is gone.

$ killall emerald
$ metacity --replace &

brings back all lost windows (also in the panel's window selector). In
my case it's 9 nautilus recycle bin windows and one azureus that I
already forgot about.

Afterwards, running "emerald --replace &" doesn't do anything. (Job done
right away.) So I have to start "compiz &".

Note: While trying all those many times, often, but not always, during
the animation of the windows towards the corners, I could see a window
flying by from right to left over the whole screen and very fast. Too
fast to see if it's a nautilus window, unfortunately. More like a quick
shadow. Gone, before you really see it.
