I'm pretty sure that I've fixed this by this fix (also backported to
dapper in debconf 1.4.72ubuntu2):

debconf (1.5.0) unstable; urgency=low

  * Notice and error out on write errors (such as ENOSPC) when saving
    databases. Should help with a lot of database corruption bugs.

Upgrades from old systems may still see it because an old debconf
frontend will be running at various points during the upgrade; as
before, use /usr/share/debconf/fix_db.pl to patch up the database.
