While you say you've checked the image, have you cleaned the CD drive? I have had this sort of problem myself in the past and it turned out to be due to a dirty CD drive lens; 10 pounds on a drive cleaning kit at a local electronics store cleared it up nicely. I recommend that anyone with this type of problem with a variety of CDs should do this before anything else. Sometimes, unfortunately, it's possible for the media test and a real live CD run to exercise the drive differently.
There are a number of squashfs errors in the log, which basically means it's completely out of the installer's control what happens; its own error-handling code might well be corrupted!
