Aha! This has now been solved. This is actually a duplicate of Bug
#63852 (or perhaps that should be the other way around?)
The other bug had the fortune of describing a simple test case ("press
ctrl-alt-del twice"), which allowed me to immediately replicate it and
thus see the problem.
And now, on re-reading this bug report, I can see that you both have
sticky ctrl-alt-del keys; a big clue being
Oct 1 15:51:12 localhost init: control-alt-delete respawning too fast,
stopped
And now I think more, the one time I managed to replicate this was while
fiddling with the shutdown sequence, and running shutdown from a shell I
had spawned within the ordinary shutdown sequence -- which fits the
pattern of Bug #63852.
I think we can mark this resolved, please upgrade to upstart 0.2.7-5
