This works on upstream OpenOffice.org 2.4.0~rc2 but fails on the Ubuntu OpenOffice.org 2.4.0~rc2-1ubuntu3. It might be a java related problem since the document has a form element in it, which iirc uses java code. If it turns out to be java related then it probably won't be fixed until we can use icedtea/openjdk.

Opening the same documents on my other machine running gutsy, and everything works fine.
