I've seen this in Gutsy and now in Hardy (I was hoping it would be fixed).
Whenever a window is maximized, at random times the title bar controls
disappear - they can often be brought back by double-clicking the title bar to
restore-down and then double click to re-maximize.
This is a huge annoyance to me - I've seen some old bugs which recommended
adding Option "XAANoOffscreenPixmaps" "true" to the Xorg.conf file, but that
did not help for me, and that's the only proposed "fix" I could find searching
around, so I decided to file the bug. I'll attach a screenshot as well.
