everything you have described there was already present in the 0.20
builds (except for "OPTIONAL: uncomment top lines to enable access for
unauthenticated streaming players" which I probably won't implement for
security reasons). There was a problem with 0.21 which has been fixed in
the last mythplugins upload.
I probably forgot to close this bug when I added authentication support
to MythWeb last year..
