Interesting, it reports ac_adapter properly, so it may be some strange bug in power-manager or ACPI reports present wrongly from time to time.
Can you Quit power-manager and start it from command line (Konsole - run "guidance-power-manager") and report back what messages are reported (if any) at the time notification appears?
