I guess upstream extension does not work, because the ubuntu-package of
the extension includes additional libraries which are needed for the
extension to work. Other extensions from extensions.openoffice.org are
working as expected. I'm not sure if it will be possible to block a
particular extension from installing.

Concerning 1:  The packages are not installed by default due to space-
limitations on installation-cd. This will not be easy to "fix". Most
people will not need every feature of ooo, therefore there was a
decision not to include everything by default.

Concerning 2: There is somewhere an open report in launchpad about this,
at the moment I do not remember the bug-number. OOO-Maintainers are
aware of this.
