Sure.  I will contribute whatever I can.  I do have a completely working
Myth machine here without the overhead of gdm or window managers and it
has been working this way for many years -- since 0.18 IIRC.  It even
worked when I used mplayer as my MythVideo player, without a window
manager.

Indeed, that has always been my feeling too.  Not default, but certainly
"selectable".

Indeed.

FWIW, I tie the green "power" button on my Hauppage PVR-250 remote with
irexec to the starting and stopping of mythfrontend.  Sort of a
"last-resort" reset in case the front end gets wedged -- which has only
ever happened due to Myth bugs, and only one in all of the time I've
been using it in fact.  It seems to be solved with 0.21 happily.
