I am experiencing a similar problem when X starts while my secondary output (projector, hdmi) is disabled. When enabling it at runtime with nvidia-settings and then choosing TwinView as display mode, I get the stupid Xinerama mode instead (complete span over both screens, every X application and WM not knowing about view ports).
