David, can you confirm the current kernel and bitness you're using with uname -a for us? I think you said you're working with Hardy now.
I just revisited the source-code based on one of the back-traces reported here and I noticed a comment in the code that might imply why this only seems to occur at start-up but not if inserted *after* the system is stable. Is that still true - that inserting the card after all the modules have loaded doesn't cause the panic? I've just completed a project to be able to deploy and build kernel modules from my PPA using DKMS packaging.
