Aha! The problem is that my systems somehow fails to initialize the card
upon power-on. It start to work after consequent warm reboot. After
powering it on I have:

It looks like BIOS issue to me. There is a new BIOS available (LF0103P)
which does not seem to have even this issue. It works for me with this
new one - try it and let us know.
