I shall look into your suggestion (Thank you) ..but this is aimed at promoting FOSS not *imposing* open standards. People who may not use OpenOffice would face trouble downloading and editing the form. On the contrary those with OpenOffice too can access *.doc file.
Also, this is for anyone who has or has not used FOSS products so that they can make the transition. All they need is interest.
