This is a bug in the hyperref package of TexLive 2012, see for example
[1]. You should do an update of your TexLive installation.
$ tlmgr update -all
I assume, that you have installed TexLive 2012 directly from upstream
like recommended on [2] with the install script [3] because I use
still TexLive 2011 as main working installation and on an other test
computer TexLive 2012 from the Quantal backports [4]. In both
installations I can’t reproduce this error. With the following minimal
example you can see the versions of all used tex packages.

The hyperref package should be after the update at least in version 6.83a.
