I don't think this is a libvirt problem. It also appears intermittent
here. In doing other testing I was looping attach and detach and found
that kvm (presumably) died, with the guest disappearing (and needed to
be started again). Attached is a (very crude) test script that can be
called with:

$ ./432154.sh <vm name> device # for attach-device
$ ./432154.sh <vm name> disk # for attach-disk

Don't use this on a VM you care about. Also, I found that kvm died late
in the guest boot process (my testing was with a 32 bit Jaunty install
on x86_64 host). Also, while the test script tries to be careful about
restoring the guest xml, it doesn't always work, and you may need to
remove the attach device by editing the XML via dumpxml/edit xml/define.
