I have a feeling it's a problem with PulseAudio. I haven't seen the exact symptoms you mention, but I have seen problems where I am listening to some music in rhythmbox and a flash movie in Firefox has no sound. IIRC, I had to close rhythmbox and firefox, then restart firefox and open the flash movie again before I got sound. Don't think I had to log out to get it working. Of course, after watching the flash movie, I couldn't listen to rhythmbox again until I shut down firefox and rhythmbox! I'm running 8.04 by the way. HTH. Sorry for the late reply.
