I have run the nvidia-bug-report.sh shell script after logging into the unity 2d desktop. I'd love to hand over the information you requested after unity 3d login but my box is unusable under 3d.
You will find the infos for the last 3d login if you scroll down the file. The last 3d login attempt is from 15th of april. Hope that helps.
