This is not an issue with 12.04. I have experienced this consistently in 16.04, 18.04 and 20.04 I have nvidia, so the common theme seems to be nvidia (however, may not even be that).
I have tried in vain to find a fix for the last 4 years and have now learned to live with it (save frequently, etc).
