This was taken from a fresh Ubuntu 9.04 install, having invoked the failsafe mode using the xorg.conf file previously attached. The exact same dialog occurs in Nexradix 1.16 (which is directly based on Ubuntu 9.04, and uses the same Xorg packages), indicating that the failsafe mode does not respect the distro name listed in /etc/lsb-release.
