Here is some more information. I checked the values below with values of both 0 and 1 for "radeon.new_pll" and "radeon.modeset=1":
1920x1080 (16:9): Wavy 1600x1200 (4:3): No Video, monitor displays message "Not Optimum Mode" shows up, as described earlier. 1680x1050 (16:10): No Video 1440x900 (16:10): No Video 1280x1024 (5:4): Wavy 1280x960 (4:3): Wavy 1024x768 (4:3): No Video 800x600 (4:3): No Video 640x480 (4:3) Wavy All of these are at 60hz refresh. I removed the KVM before doing this test.
