I tend to agree with the OP that this is unintuitive. I'd recommend a checkbox on the "Change Default Grid" box, marked "Update current image?" or something along those lines.

Ubuntu 14.04 x86-64, gimp 2.8.10-0ubuntu1 (from official repo). Steps to reproduce: Go to "Edit" -> "Preferences" menu -> choose "Default Grid" -> change "Spacing" -> click "OK". If grid is not enabled, go to "View" -> "Show Grid" menu and check it to show grid. The grid spacing / cell size is equal to old settings, not to new after changes. Only if I close gimp and open again it apply new grid spacing and show grid with new cell size. Expected: When I change grid spacing in preferences and click "OK", Gimp should immediate apply changes and change grid to new cell size without program restart. Grid Offset settings works same: can't apply without program restart.
