edquota 111 and change block and inode soft and hard limits all to 0 (zero).
I just used edquota to create a quota by uid, and to delete it, in just that way (Solaris 9). Since the used numbers for blocks and inodes appear to be 0 (zero) in your example, that should be no big deal.
