These changes instead only ignore exceptions that occur after the client stops 
the service, assuming that the ServiceShutdownException is to be expected then.
Some tests had to be corrected (by adding to AllowContinueOnFailure) and the 
AdvancedFixedFlowController has a new AllowDropOnFailure option so that child 
CASes with errors can be quietly dropped from the flow and not cause exceptions 
to be returned to the client.
CAS flow tracing has also been added to the flow controller.

One test now fails, testJmsServiceAdapterWithGetmetaTimeout, but I don't think 
it's testing what it should be testing.
