It could also be that the customer's access provider - CenturyLink, you said, 
Mark? - has routed them a block of PA which has history of being in these 
places.  There are tools provided by the likes of Google for providers to 
geocode IP address space e.g. https://isp.google.com/geo_feed/

"You may provide Google with an IP geolocation data feed to help improve the 
location accuracy of Google products and services."

Example: https://isp.google.com/static/downloads/example-geo-feed.csv

But it does require the IP address "owner" to use them and keep such feeds up 
to date with some automation/exports/etc.
