This may indicate a bug in the Promise BIOS.

Did you try it using "No large disk support"?  This should cause
FreeDOS fdisk to emulate the version from MS-DOS 6.

Have you tried flashing the latest BIOS into the Promise controller?

If neither one of these helps, the next step will be to submit a bug
report to the FreeDOS folks...
