I decided to migrate to a different FC5 box and in the meanwhile haven't
had the time to circle back with tech support to continue to
troubleshoot.

In the meantime here's the workaround I use:

First, delete the SlimServer database manually

rm /usr/local/slimserver/.slimserversql.db

Next, go into the Server settings in the web interface, and change the
current music folder to be blank.

Then, go to Server settings and select the option to clear the current
database and rescan.

Next, change the current music folder to be the real location of your
music.

Finally, go to Server settings and select the option to clear the
current database and rescan.

To ensure that the database is being rebuilt:

ls -al /usr/local/slimserver/.slimserversql.db

and you should see the file. Issue the command a few times and the file
should be growing until the entire music folder has been catalogued.
