I just tested on CentOS 4 and it works fine for me.

The RPM requires the CentOS mysql-server package be installed. When SC
is started, it runs it's own instance of mysqld using that system
binary. There is no extra startup script for mysqld. There is no need
to manually configure mysqld or move/create any db files.

Can you double check this? The symptoms you describe are very similar
to what happens with SELinux enabled.
