I have a similar problem. On some folders songs are missing or i see no covers. Also get the message in the server.log that the file couldn't be opened. The problem is gone when i disable MusicIP and let squeezecenter scan the music library again. At the moment i think it's maybe because i use a external drive that's formated with ntfs and the problem is because of the different character sets.
