Looks like you are not connecting to your MySql database for some reason. If I were you I would go to the Wiki (http://wiki.slimdevices.com/index.php/Linux_Installation_Guide) and learn how to remove and reinstall SBS. Be sure you download and install the RPM (http://www.mysqueezebox.com/download) as there appears to be a few problems with the repositories at the moment.
Everything should set itself up and create the Slimserver user.
