I've seen this happen before on my install, although I'm not really sure what caused it. If I remember correctly, to correct it I opened the server page with another skin -- something like http://<server name>:9000/fishbone. This allowed the left menu to load correctly. I then switched back to the default skin and it seemed to be fine. (Also might have had to clear browser cache.)
