That box is used when you have a board with input capabilities. (i.e. Hifiberry Dac+ADC) It is not needed for bluetooth devices.
Streamer is a bit finicky with the order things happen. Connect your phone, and start playing music, then try to play the stream. If you try to play the stream before the bluetooth device is fully ready, it can cause the stream to hang......then you have to wait a while for it to timeout. Sometimes this matters when you try to create the favorite too.
