Does anyone know how to make a boot CD/DVD from scratch?

I know you can get an image, and instructions on tools for creating one, out on 
the internet, but what does it actually take, to make a CD/DVD bootable under 
OS X?

I have an image that I burned, and its not bootable.  It was created using 
Nero, which is only PC.  I think I can follow some inctructions to create a 
bootable image, and then copy these files over to that image, but perhaps 
there's a simpler solution.

And yes, if you're wondering Nero images do seem to be ISO images...with a 
different extension...so you can change the extension to ISO, to access it.
