Scott already mentioned Nagios, which is a great solution
if you're looking for monitoring and alerts if your usage
exceeds a certain threshhold, or if a server or service becomes
unavailable.

For tracking over time, I'd suggest you look at Munin.  Like
Nagios, the base Munin system isn't written in PHP, but you can
use it with PHP and use PHP to write custom plugins for it.
Munin also integrates well with Nagios, if you plan to use both.
