Mike,

check and see if you have this dll in the php dir or sub dirs on the server:

ntwdblib.dll and then check and see if you have one under
C:\windows\system32 or equivalent for your windows version.  If you have one
under the system32 dir then i would suggest moving the one under php out of
there and put it in a backup directory or someone outside of php. Then try
again and see if that works for you.
