Bowel Management Systems

The consequences of the inability to manage fecal output can range
from embarrassment to death. Most individuals take this normal body
function for granted and only occasionally experience disruptions
(e.g., diarrhea, constipation), but this situation can change
drastically when an individual is hospitalized. Overnight,
individuals with previously normal defecatory function can suddenly
find themselves incontinent and forced to use a bedpan or defecate on
themselves as a result of medications and/or restriction to bed rest.
The causes for bed rest restrictions of hospitalized patients are
many, including accidents resulting in head, spinal cord, neck, back,
pelvis or femur injury, and stroke or coma as a result of an
overwhelming infection or liver failure.

To complicate the situation further, there is a segment of this
patient population with open wounds on the perineum from burns,
bedsores or necrotizing fasciitis. These wounds are at risk of
contamination with each uncontrolled bowel movement. This problem is 
so serious that some of those suffering from perineal wounds require
a diverting colostomy in order to ensure no fecal contamination
occurs during the healing of the wounds. 

Besides complicating existing wounds, fecal leakage and diarrhea
increase the risk of perianal excoriation and bedsores. Clearly, the
burden and cost of preventing fecal incontinence from negatively 
impacting morbidity and mortality rates is substantial for the health
care system. Yet to date, no viable means of addressing this serious
issue has been commercialized-- all the more interesting in light of
the fact that urinary management systems are readily 
available and outinely used.

Indwelling Fecal Management System 
The Indwelling Fecal Management System (IFMS) is designed to safely
and reliably divert, collect and contain potentially harmful and
corrosive gastrointestinal waste from bedridden and immobilized
patients. The first generation product for bowel management has
already been successfully clinically tested in over 50 humans
(indwell time ranged from 3 to 60 days with a mean indwell time of
12.5 days). The data from the first 32 patients has already been
published in the prestigious American peer-review surgical journal
Diseases of the Colon and Rectum (August 2001, Volume 44, Number 8).
The need to divert, collect and contain stools is of particular
relevance to bedridden or immobile patient populations experiencing
severe or chronic diarrhea, constipation, impaction, incontinence or
those at risk of serious infection resulting from fecal exposure and
contamination such as burn, perineal, obstetrical, lower limb or
pelvic orthopedic or urological surgery patients.

As critical as the diversion of stool is from a clinical perspective,
the containment and disposal of feces is equally critical from a
hygienic perspective. The IFMS is designed to facilitate the quick
and easy collection and containment of all the patients' waste while
reducing the possiblity of caregiver exposure to feces and odor.
Disposal of waste can easily be achieved by the rapid disconnection
of the collection bag from the indwelling catheter.

As of May 3, 2002, Zassi won FDA clearance for the Bowel Management
Systems product and anticipates that it will be available for sale by
the fourth quarter of 2002. The company is presently in negotiations
with various leading healthcare firms to establish a worldwide sales,
marketing and distribution network. Zassi has a similar arrangement
for its Ostomy Management technologies with ConvaTec, a Bristol-Myers
Squibb company.
