What speed is the device? Which sort of controller is this on? (UHCI, OHCI, 
EHCI, XHCI)

Does the host attempt multiple OUTs, which are NAKed, or is there nothing?

If there are OUTs, I’m not sure the host is doing anything wrong, you may need 
to fix the device.
