For those following this thread, upgrading to OS X El Capitan 10.11.1 seems to 
have eliminated the issue altogether with no change in USB hubs, devices, or 
configuration.

At this point, my Mac Pro (Late 2013) is now as stable as my former Mac Pro 
(Late 2010).
