You probably know these bits already - they're unfortunately also not 
X-platform:

1. Have you tried   sips    from the command line on OSX? 

e.g.   sips -s format png mypdf.pdf --out myimage.png

2. Maybe Irfanview on Windows. Apparently it has command-line options.
