I think it's not enough just to have an index, it has to serve a purpose as well. I do use the printed docs, but the ability to search the online docs is a great tool. And pdf indices seem to serve as a nice middle ground, allowing for thematic groupings as in a table of contents and hyperlinking to content.
Agree, but darn it's a lot of work. I made the task-oriented index for SuperCard 2.5's documentation, it came to about 15-20% of the total cost. And people still complained that it wasn't complete enough. :)
